Hello Nat,
The problem is that starting with PHP 4.4.0 and PHP 5.0, the way variables are returned has changed.
The problem is that the latest version of MXSurvery was released before these PHP versions were released, and there is still some code that is not handled correctly on your PHP version.
I will submit this bug report on our internal bug-tracker to be fixed in a later release of this product. Thank you for reporting it.
Regards,
Florin CARLIG

Can you offer a fix? Do I need to wait for an update?

Hello Nat,
Yes, I'm afraid you're gonna have to wait for a new release for this product.

Warning: imagepng(): Unable to open 'charts/32.png' for writing in /users/webusers/jbsb/buchanan-assoc.com/htdocs/survey/includes/mxsurvey/SVGraph.inc.php on line 150
This is the next problem I have, which may be a problem with file permission on the server. Do I need to talk to my server admin about this, or is this because of the PHP5 issue?
Thanks!

Hello,
Yes, I think this is an issue for which you'll have to talk to your host. The error indicates that you don't have write permissions on the charts folder.
